PAYMENTS AND INSURANCE
If you have insurance, fill out The Lactation Network information to see if you’re covered.
Payment is due at the time services are provided.
A $50 fee will be assessed for all appointments cancelled without a 12-hour notice.
I accept Zelle, Venmo, and cash.
Note: Lactation services are typically HSA/FSA eligible. Check with your provider for specifics.
For out of network services I will provide a superbill for you to seek reimbursement from your insurance company. Reimbursement from your insurance company is not guaranteed.
If you are not covered with Lactation Network, most insurances still cover lactation, but you would privately pay and then submit the super-bill I will provide you.

RESCHEDULING AND CANCELLATIONS
I often schedule clients back to back, so being on time is so appreciated. If you are more than 15 minutes late, we may need to reschedule. If you are a no-show, you will be charged $50. Please give at least 12-hour notice to change or cancel appointments.
I will bring with me the paperwork and a care plan we will personalize together.
